If you would like to become a CHS Athletic Booster member, please complete the membership form below and return it to a CHS Athletic Booster Officer or the CHS Athletic Director.

Membership Dues are $10.00 per person annually.
If you are interested in serving on the CHS Athletic Board for the upcoming school year, please complete the application below.
You can turn it in to our current Athletic President (Ms. Kim Griffis), Athletic Director (Kyle Pycraft), Mr. Martin (Asst.Principal) or email a copy to : Cummings.boosters@hotmail.com
